After-care Program

After a resident graduates from our transitional housing program, they enter our after care program. Our case manager continues to work with the graduate, as they move on to independent living. This includes home visits, case management sessions, and community events. The graduate can continue to utilize most of the services at EGH and many have come back to volunteer.
